Oshkosh Chapter Mentor Award Nomination Form

The Women in Management Oshkosh Chapter Mentor Award is presented annually to one or more individuals (male or female) who have mentored you or other women. The candidate could be either a personal mentor to you or someone who has reached many. Attached is an impressive list of former recipients. Past Mentor award recipients may be nominated more than once. Take a few minutes and think about those who have ‘made a difference’ in your success or continue to mentor you in your career. There is no action too small to be recognized if it helped you succeed.
